What is a reverse shoulder replacement?
The reverse shoulder replacement is an artificial joint in which – unlike the anatomical version – the ball is fixed to the socket of the shoulder and the socket sits on the upper arm. This reversal of the principle allows the shoulder to function even where the rotator cuff is damaged or ruptured.
Because the mechanics are changed, the deltoid muscle takes over the function of the missing tendons. This technique is used above all with irreparable tears of the rotator cuff, with complex fractures and with osteoarthritis of the shoulder where the muscles are insufficient.
Indications for a reverse shoulder replacement
- An irreparable tear of the rotator cuff
- Osteoarthritis of the shoulder with cuff arthropathy
- A complex, comminuted fracture of the upper humerus
- Revision after loosening or malfunction of an anatomical replacement
How the operation proceeds
- General or regional anaesthetic
- Deltopectoral approach to the shoulder
- Removal of the damaged part of the joint
- Implantation of the glenosphere (the ball) on the socket of the shoulder
- Anchoring of the humeral implant with its polyethylene socket
Rehabilitation after a reverse shoulder replacement
Phase I (0-4 weeks): protection and passive movement
- Wearing a sling for 3-4 weeks
- Cooling, pain management, lymphatic drainage
- Passive mobilisation exercises (above all elevation to 90°)
- No loading and no active movement of the shoulder joint
Phase II (5-8 weeks): active-assisted mobilisation
- Introducing active movement with assistance
- Strengthening the deltoid and the surrounding muscles
- Avoiding overhead work and impact loading
Phase III (from week 9): functional loading and everyday life
- Extending the range of movement (aim: 120-130° of elevation)
- Strengthening the shoulder muscles in everyday life
- Return to work and sport after discussion with your doctor
